Nail psoriasis

Nail psoriasis is a frequent and often underestimated manifestation of psoriasis, affecting up to 60% of patients and up to 80% of those with psoriatic arthritis. In a smaller subset, nail disease may present in isolation. This presentation will explore the distinct clinical signs of nail matrix, bed, and fold involvement, and highlight the critical role of clinical examination, dermoscopy, and imaging in diagnosis. Given the association between nail psoriasis and psoriatic arthritis, early recognition is essential to prevent joint damage. Recent advances in imaging modalities such as HFUS, RCM, and OCT for the early diagnosis and monitoring of nail psoriasis will then be discussed.
Treatment remains challenging due to the anatomy of the nail unit and slow nail growth, requiring long-term management strategies tailored to the patient’s disease severity, comorbidities, and quality of life impact.
This presentation will review topical, intralesional, systemic, biologic and small molecules treatments, outlining indications based on the site and severity of nail involvement. Through this comprehensive approach, dermatologists can optimize outcomes for patients suffering from this complex and often distressing manifestation of psoriasis.
