Use of mohs surgery in non-keratinocytic tumours

Mohs micrographic surgery (MMS) is the gold standard for basal and squamous cell carcinomas and is increasingly used for non-keratinocytic tumors, including melanomas like lentigo maligna melanoma (LMM) and melanoma in situ (MIS). MMS offers precise cancer removal, but its use for deeper melanomas is still controversial and requires more research.

Dermoscopy of melanoma

Histology is the gold standard for melanoma diagnosis, but excising all suspicious lesions is costly. Imaging methods like RCM and LC-OCT are less invasive but expensive, limiting their use. Dermoscopy, however, is affordable and effective, improving diagnosis while reducing unnecessary excisions. This lecture uses images to teach key dermoscopic features of melanocytic lesions and melanoma, covering common pitfalls and special areas like the face and mucosae.

Pediatric cutaneous melanoma is rare, comprising 1% of childhood cancers. It often differs from adult cases, sometimes linked to large congenital nevi. Its rarity and unique traits complicate diagnosis and treatment, emphasizing the need for more research to improve outcomes.

Psoriasis and lichen planus in nails

Nail psoriasis and lichen planus are chronic conditions affecting nails and quality of life. Psoriasis shows pitting and onycholysis, possibly needing rheumatologist referral for arthritis. Lichen planus causes ridging and potential permanent damage, requiring early treatment. Treatments include topical and systemic therapies, with future options like JAK inhibitors.

Psoriasis – Therapeutic drug monitoring in your practice?

Psoriasis treatments now include biologics targeting TNF-α, IL-17, and IL-23, but patient response varies. Some patients do not respond or lose response, while others achieve near-complete skin clearance, allowing dose reduction. Clinicians are optimizing doses empirically and investigating this for newer biologics. Therapeutic drug monitoring (TDM) measures drug levels and anti-drug antibodies, helping guide treatment based on exposure-response relationships, which have been identified for newer biologics.

Population differences in psoriasis

Psoriasis affects 125 million people globally, with variations in symptoms and treatment based on skin type, age, gender, race, and ethnicity. Education on these differences can improve care and reduce stigma. Psoriatic arthritis is more severe in darker-skinned individuals. Genetic and environmental factors influence the disease’s severity and should be considered in future research.
